This is a personalised programme rather than a one-size-fits-all approach. You and your individual injury is unique therefore your recovery plan will be based on you and your goals. Whether your goal is to sit more comfortably, lift with confidence, keep up with family life or return to demanding sport, we’ll focus on steady progress that feels relevant and achievable.
Why the 4-STEP Programme Works
The 4-STEP Programme is designed to help you feel supported from the very beginning. We start by listening to what you are experiencing, helping you make sense of your symptoms and finding gentle, practical ways to make things feel more manageable. As you progress, the focus gradually moves towards comfortable movement, growing strength and everyday habits that help you feel more confident.
We know that recovery does not always follow a straight line, and that is completely normal. Each stage is flexible and shaped around you. Your physiotherapist will work alongside you, adjusting the pace and level of challenge to suit your symptoms, lifestyle and goals, so you can move forward steadily without feeling rushed.
You will be an active partner in your recovery, but you will not have to work it out alone. We’ll explain each activity clearly, show you how it connects to your goals and give you the support you need to practise with confidence. Whether you want to manage the working day more comfortably, return to sport or enjoy time with family, your programme will stay focused on what matters most to you.
Step 1: Settle Pain and Irritation
We’ll begin with an assessment where we take time to understand your past injuries, health conditions and lifestyle so we can tailor your treatment safely and effectively. We assess strength, movement, flexibility and any painful or restricted areas. These tests help us pinpoint what’s going on and how best to treat it. Whether it’s bending, lifting, walking or sitting comfortably, we look at how your injury affects your everyday life — and what needs to change.
Hands‑on treatment is often the starting point. This may include soft‑tissue massage to relax tight muscles, joint mobilisations to improve stiff segments, or gentle nerve‑gliding techniques if irritation is affecting the nerves. These approaches help reduce protective muscle guarding and restore smoother movement.
We’ll review how you respond and adjust your plan as needed. You won’t be expected to push through pain or make changes too quickly—the aim is to help you feel supported, more comfortable and ready for the next stage.
Goal: To help pain and irritation feel more manageable, improve your understanding of your injury and give you a clear, reassuring path towards comfortable movement.
Step 2: Train Movement
Once your symptoms feel more manageable, we’ll help you become comfortable and confident with movement again. We’ll look at the everyday actions that matter to you—such as bending, walking, sitting, lifting or returning to work—and practise them in a way that feels safe, controlled and achievable.
Good movement isn’t just about flexibility — it’s about teaching your spine, hips and core to work together smoothly and confidently. Physiotherapy helps retrain movement patterns so your body can move without stiffness, hesitation or pain.
Your physiotherapist will guide you through movements that are suited to your current ability. We’ll start at the right level for you, explain what to expect and gradually add variety or challenge as your confidence grows. You won’t be expected to move perfectly or push through pain. We’ll help you notice what feels comfortable, adjust when needed and build trust in your body one step at a time.
Goal: To help you move more freely and confidently, with practical skills you can use every day.
Step 3: Enhance Strength
As your movement becomes more comfortable, we’ll help you build the strength and control needed to support your body with confidence. We’ll focus on the activities that matter to you, whether that means managing everyday tasks, meeting the demands of work or preparing to return to a favourite sport or hobby.
Building strength is one of the most important parts of long‑term recovery. Once pain has settled and you begin to move easier, physiotherapy focuses on strengthening the muscles that support your body so they can handle everyday movement, work demands and sport without becoming irritated again.
This structured approach doesn’t just make you stronger — it improves coordination, balance and endurance, helping you move more efficiently and reducing the risk of future flare‑ups. It’s the phase where your body becomes truly resilient.
Goal: To help you feel stronger, more capable and ready for the activities that are important to you.
Step 4: Protect Through Prevention
Once your injured area is moving well and feeling stronger, physiotherapy shifts towards long‑term protection — helping you stay better, not just feel better. This phase focuses on building the habits, strength and movement patterns that keep your body resilient in everyday life.
We will guide you through exercises that improve the stability of your core and hips, giving your body the support it needs during lifting, bending, sitting and sport. We also refine how you move, teaching efficient techniques that reduce unnecessary strain on sensitive joints and discs.
Just as important are the small daily adjustments that make a big difference over time. This may include advice on workstation setup, pacing activities, managing load through busy periods, and recognising early warning signs before they turn into a full flare‑up.
By combining strength, movement retraining and practical lifestyle strategies, physiotherapy gives you a toolkit that protects your back day‑to‑day — helping you stay confident, active and pain‑free long term.
Goal: To help you maintain your progress, reduce the risk of future flare-ups and feel confident caring for your back over the long term.
